Member Eligibility
Medica Member eligibility will be verified through the MedImpact on-line claim processing system. You may also verify eligibility by calling:
- the MedImpact Pharmacy Hotline at 1-800-788-2949
- the Medica service line number listed on the back of the Member's Medica Identification Card ("ID Card")
- if Member is a Medicaid Member, the Minnesota State eligibility line at 1-800-657-3613. Medica's Provider I.D. is 405713950.
Under no circumstances should a Member whose eligibility has been verified, be denied a Pharmaceutical Service (subject to Pharmacy's professional judgement), or be asked to pay more than is due under the terms of the Pharmacy Participation Agreement with Medica.
Relationship Codes and Eligibility Clarification CodesThe following relationship codes are used when adjudicating claims submitted by Pharmacy.
Relationship Codes
00 = Subscriber
01 = Spouse
02 = Child
Additional children and/or dependents may be identified by 03, 04, etc.
