Medica is now accepting credentialing applications submitted through ApplySmart, a web-based credentialing application completion and submission tool.

Currently Registered
If you are currently registered to use ApplySmart, you can submit an application for credentialing with Medica or any other entity online. Go to ApplySmart.

Not Yet Registered
Contact the Minnesota Credentialing Collaborative (MCC) to start taking advantage of this electronic submission tool. Go to the Minnesota Credentialing Collaborative.

Medica will still accept the Minnesota Uniform Credentialing Application below. Applications submitted on a form other than one of these will be returned unprocessed.

Please note: The submission of a completed credentialing application does not constitute a provider contract with Medica. To learn more, go to Join Our Provider Network.


Submitting Applications to Medica

Submit all credentialing applications through ApplySmart or send to:

Credentialing applications submitted to Medica must include an Authorization and Release naming Medica or Medica Health Plans as the entity.
