Minnesota Uniform Credentialing Application


Medica is now accepting credentialing applications submitted through ApplySmart, a web-based credentialing application completion and submission tool.

If you are currently registered to use ApplySmart, click the following link to complete your application for submission to Medica and any other entity you are seeking credentialing with:

Medica will still accept the Minnesota Uniform Credentialing Application below. Applications submitted on a form other than one of these will be returned unprocessed.

Please note that the submission of a completed credentialing application does not constitute a contract.  Please contact Medica's Network Management for more information on the process: NetManQuest@medica.com

Credentialing applications submitted to Medica must include an Authorization and Release naming Medica or Medica Health Plans as the entity.
