Every effort is made to respond to general coding questions with answers that are complete and accurate at the time. However, it should be noted that policies are subject to change. General coding recommendations may not always apply to specific claims due to contractual agreement, member certificate of coverage, or other extenuating factors. In addition, claim policies and procedures such as timely filing may be applicable.
